Step 1: Read the Information Statement.

Step 2: Complete the Investment Application in which you state the terms of your investment and provide information regarding investment choices and re-investment alternatives.

Step 3: Sign the Investment Management Agreement.

Step 4: Return the completed and signed documents along with your investment check to:
